Issued for operational service at any time between 11th June 1943 and 8th May 1945 in Sicily and Italy. The award also covered operational service in the Agean, the Dodecanese, Corsica, Greece, Sardinia, Yugoslavia and Elba. However, service in Sicily after 17/8/1943, in Sardinia after 19/9/1943 and in Corsica after 4/10/1943 did not qualify. This star was issued in addition to the other stars, so no bars were awarded with it.

A six pointed bronze star with the GRI/VI monogram in the circular centre, surmounted by a crown. Inscribed 'THE ITALY STAR'.

The ribbon is five equal stripes of red, white, green, white and red (the Italian national colours).
